The Personnel Committee members annually evaluate the independent contractors who serve in the roles of Treasurer, Executive Secretary, and Deltion Editor.

• Appraisal State Treasurer

• Appraisal Executive Secretary

• Appraisal Deltion Editor

INSTRUCTIONS TO ACCESS & TYPE ON THE FORMS:

1. Click on the link to the form -- once opened, click on File, Download

2. The Microsoft Word file will download to your computer.  

3. You can open the file, type into the document, and then save the document.

4. You DO NOT need to "Request Edit Access"
