INSTRUCTIONS TO ACCESS & TYPE ON THE FORMS:

1. Click on the link to the form -- once opened, click on File, Download

2. The Microsoft Word file will download to your computer.  

3. You can open the file, type into the document, and then save the document.

4. You DO NOT need to "Request Edit Access"
