Achievement Award

2024 Application Deadline:  January 15, 2024

The application must be completed online using the Google form linked below.  Upon submission of the online form, submit the most recent photo of the nominee/applicant to Lisa Taylor, Chairman of the Achievement Award Committee.  The form and photo must be received by the deadline.

• All questions require a response.  If information is not available for response, insert N/A or None. 

• Applications are based on the point-system.  For example: Chapter Committee Member = 1 point; State President = 6 points.

• Ensure the information is specific to the request.  Stating whether a person was a committee member, or a committee chair will make a difference in the number of points awarded.

• Section 6 will be considered subjectively and may be used as a tie breaker.  Anecdotal comments are encouraged to illustrate the merits of the Nominee.

Qualifications for Achievement Award Nominees:

• Member for at least seven (7) years

• Contributed to the advancement, growth, and development of Louisiana State Organization.

• Not presently a DKG state officer or paid personnel

• Nominee is not a previous recipient.
