INSTRUCTIONS TO ACCESS & TYPE ON THE FORMS:
Click on the link to the form -- once opened, click on File, Download
The Microsoft Word file will download to your computer.
You can open the file, type into the document, and then save the document.
You DO NOT need to "Request Edit Access"
Click the link above to submit member news and concerns and request a sharing/caring gram be sent.

If you would like to download the forms in Microsoft Word format to allow you to type into the forms, click on the folder graphic to the left to access the "Membership" Folder. You will see the list of forms and can click the link to download them.
